Attach a form to a terminal option when the ticket needs structured details before anyone responds. Forms cannot run on branch options.

Turn on the form

Select a terminal option and enable Ask questions first. The form editor opens with one required long-answer question.

Name the form

Enter a short Form title. Discord displays it at the top of the pop-up.

Write the questions

Add between one and five questions. Every question needs a label. Add an optional hint, mark it Required when the customer must answer, and enable Long answer for paragraph input.

Review the result

Select Done, then open Discord preview and choose the option to inspect the pop-up before saving.
Only the ticket opener can submit the form. Discord blocks submission when a required answer is empty. Submitted answers are saved with the ticket and shown to the support team in the inbox. Handling begins after submission. For AI answers, the completed fields are combined into the customer’s first question, so the AI can answer without asking them to repeat the details. For Your team, the answers are included as context for the handoff. Optional unanswered fields are ignored when building the message. Keep forms short. Ask only for details that change the answer or routing, and use one long-answer field instead of several overlapping questions.
